Description: In this course, the Coordinated Entry team at All Chicago will walk through the CES Transfer Policy and the process for requesting Internal and External Transfers. Additionally, attendees will learn and practice how to determine whether a household meets HUD's definition of chronic homelessness, utilizing Chicago's Chronic Homeless Verification Packet, as this is an essential part of all transfers to Permanent Supportive Housing (PSH).

Recommended Audience: Any providers who submit CES Transfer Requests or who support participants in the Transfer process are encouraged to attend.
