02.13.25 Completing Verifications of Disability as a Supporting Provider: End-to-End User Training

Description: Support Providers in Chicago’s CoC serve the critical role of helping participants prepare to exit homelessness and enter housing. Many participants of CoC programs live with a HUD-defined disability, but do not have the documentation required to receive services appropriate for their needs. In this training, we will review the importance of providing Verifications Of Disability (VODs) to eligible participants, tools you and your team can use to observe disabilities, and the process for completing and uploading the VOD document. By ensuring that participants have all the documentation which they are eligible for, we aim to improve their ability to access the most supportive level of care possible and reduce returns to homelessness.

Attendees will learn about:
• The importance of VoDs in the housing process
• Tools to observe HUD-defined disabilities in program participants
• Practical skills to explain the VoD to a participant
• Connecting with licensed providers to complete the VOD for a participant
• Verifying disabilities in HMIS
• Uploading VODs in HMIS

Recommended Audience: Any supporting provider in the CoC (including outreach workers, shelter staff, and drop-in staff, direct service case managers, and program coordinators)
